- All-American Cafe - free dessert
- Amato's - Free Amato's Original Sandwich. Current locations include Maine, New Hampshire, and Vermont.
- Applebees - Free dessert on your birthday (just let them know when ordering)!
- Atlanta Bread Company - free cookie on your birthday. They seem to have locations spread out through the country, not just in Atlanta.
- Baskin-Robbins - free ice cream!
- Black Angus Steakhouse - receive a free dessert for signing up, and a free steak dinner on your birthday! Locations in the western states.
- Bob Evans - free kids meal on their birthday (12 and under).
- California Pizza Kitchen - free kid's meal during the month of their birthday (10 and under).
- Cheesecake Factory - free scoop of ice cream with a lit candle and singing by the staff
- Cold Stone Creamery - free ice cream!
- Cracker Barrel - free dessert
- Denny's - free kids meal and dessert (10 and under)
- DQ - register to receive a coupon for free ice cream on your birthday
- Famous Dave's - free entree
- Friendly's - more free ice cream!
- Frosty Paws - free dog treats for your dog on their birthday. Ruff!
- Noodles and Company - free dish on your birthday
- Old Country Buffet - free meal on your birthday
- Quiznos - a free sandwich just for signing up, and a free cookie on your birthday
- Red Lobster
- Red Robin - free burger on your birthday
- Sephora - sign up in the store, or on-line and receive a free gift within two weeks of your birthday. This year I receive a bottle of bottle of glittery vanilla cupcake scented body wash / bubble bath. A minimal purchase order is required to receive the gift through their Web site, but you can also walk into their store and receive it with no strings attached!
- Silver Diner - I didn't see it mentioned on their Web site, but for my birthday I received a coupon for a free entree and dessert with purchase of another entree. The coupon was good for about a month, so you can save it and go out when you like.

Recently a "Free Dessert on your birthday" restaurant was discoved by a group of my friends - Landry's Seafood House. The dessert was a scoop of ice cream with unsalted tortilla chips with carmel/cinnamon topping. Quite tasty!
